Pass it and your USDOT becomes permanent. Fail and your authority can be revoked. Most failures are paperwork, not safety. Here are the five categories auditors hit first.

1. Driver qualification files

Under 49 CFR Part 391, every CDL driver needs a complete DQ file: application for employment, motor vehicle record (MVR) checks, road test certificate, medical examiner’s certificate, drug-and-alcohol pre-employment test, annual MVR review, and a signed annual list of violations. Auditors will pull two or three random files and check every page.

2. Drug-and-alcohol testing program

Per 49 CFR Part 382, you must enroll every CDL driver in a DOT-compliant testing program — typically through a consortium / TPA. Auditors will ask for the consortium membership letter, your designated employer representative (DER) appointment, the random selection lists, and the FMCSA Clearinghouse pre-employment query results for every driver hired since January 2020.

3. Hours-of-service records

Six months of ELD logs and supporting documents (toll receipts, fuel receipts, dispatch records). Auditors look for false logs, 14-hour rule violations, missing certifications. ELD-exempt operations need short-haul exception logs or paper logbooks with the same depth.

4. Vehicle maintenance and inspection records

Annual DOT inspection reports for every power unit and trailer, daily driver vehicle inspection reports (DVIRs) showing defects and repairs, and a maintenance file per vehicle. Missing annuals are one of the most common audit findings.

5. Accident register

Even if you’ve had zero accidents, you need an accident register on file confirming so. Per 49 CFR § 390.15, the register lists DOT-recordable accidents (fatality, injury requiring transport, vehicle towed from scene). Auditors specifically look for this — a missing register is treated as failure to maintain records, not as zero accidents.

The pre-audit prep is more important than the audit itself. We pull every record in advance, identify gaps, and fix them before the auditor arrives.
